Kindred Transitional Care & Rehab-Milwaukee, formerly known as Mount Carmel Health and Rehabilitation Center, is a nursing and rehabilitation center that serves individuals in Greenfield and surrounding areas. The center provides residents with short- and long-term care, and also offers outpatient rehabilitation services.

A variety of medical and therapeutic services are offered, including specialty recovery programs, respite care, hospice care and speech, physical and occupational therapy.
