Kindred Transitional Care and Rehabilitation, formerly known as Mt. Carmel, announced it was closing sometime next year earlier this month.
Days later, the Milwaukee Journal Sentinel reported the facility at the corner of 60th Street and West Layton Avenue was found to have committed 23 federal violations during a 350-page inspection conducted by the Department of Health Services.
