Greenfield Health Department Seeking Community Input

Resident input important to shaping community health priorities.

The is working to complete a community health assessment within the next few months in preparation for the next five-year community health improvement plan.

The community health assessment and improvement plan are both mandated by Wisconsin Statutes every five years, and build upon the state’s decade-long health plan, Healthiest Wisconsin 2020: Everyone Living Better, Longer.

"Feedback from residents is one of the most important steps in the assessment process," Darren Rausch, Health Officer/Director, said in a statement. "Resident input is invaluable in shaping the community health improvement plan to maximize the benefit for everyone."

The community health survey is accessible online here, and in paper form at the Greenfield Health Department. The survey collects information on a variety of topics including risky health behaviors, community health and environmental health concerns. The survey will take about 5-10 minutes to complete. Survey responses and demographic information will remain confidential. The survey will be available to residents through the end of October.

Additionally, the Health Department is seeking resident representation and input in the health improvement planning process (scheduled to begin in October). This process will include two meetings with key community leaders and residents to prioritize the health needs of the community.
